Most AI tools that land in clinical practices do not fail because the technology is bad. They fail because no one asked the right questions before switching them on.

There is a pattern. A practice owner sees a polished demo. The numbers look compelling. They sign up. Three months later the tool is either switched off or quietly creating more admin than it saves.

This is a sequencing problem. And it is almost entirely avoidable.

The demo shows ideal conditions. Your practice is not ideal conditions.

AI vendors are very good at demonstrating their products when everything is configured correctly. What the demo does not show is what happens when your call comes in mid-consult handover, your head nurse's informal Tuesday policy conflicts with the system's booking logic, or a distressed client needs to feel heard before they will engage with scheduling at all.

These are not edge cases. They are Tuesday.

'Plug and play' is a sales phrase, not a strategy.

Clinical environments are structurally different from the sectors where most AI tools were built: e-commerce, hospitality, banking. Those environments have stable, well-documented customer journeys. Yours does not. A single inbound call might involve triage, species-specific protocols, post-operative follow-up, and emotional support, often in the same conversation.

Software that has never spoken to your team has no way of knowing that.

The questions to ask before you buy anything

  • What actually happens when your phone rings? The real version, not the theoretical one.
  • Can you categorise your inbound demand by reason, urgency, and outcome? Most practices cannot.
  • Where are clients currently experiencing friction in the phone journey?
  • What does your front-of-house team actually think about AI? Have you asked them?

That last question is the one most practices skip. A team that feels threatened by a tool will work around it, consciously or not. Successful AI adoption is a change management exercise as much as a technology one.

Deploying a tool is not the same as embedding a capability

Deploying takes days. Embedding takes months. The difference is whether someone with both clinical context and technical literacy is holding the process together: mapping workflows before configuration, involving the front-of-house team in design, and measuring performance from day one.

Most practices do not have that person in-house. Most vendors do not provide them either.

One question that separates good vendors from the rest

Do not ask what their product does. The demo will tell you that. Ask:

"What does your implementation process look like, and what does success look like at month three, six, and twelve?"

If the answer is vague, treat that as a data point.

Where to start

Not with a vendor evaluation. With a single conversation between your practice manager and your front-of-house team, built around three questions:

  1. Where do we lose clients in the phone journey today?
  2. Where do we create unnecessary work for ourselves?
  3. If we could change one thing about how we handle inbound demand, what would it be?

Those answers will tell you more about AI readiness than any feature comparison document.

Technology follows strategy. The sequence matters.
